Turn the README into full operator and contributor documentation.

Scope:

  • Installation, Action setup, token scopes, providers, guidance, config schema, checks, forks, troubleshooting, privacy, and local runs.
  • Example configuration and review output.
  • Document how to add a provider and the advisory/human-review boundary.

Acceptance:

  • A new repository can configure the tool without reading source code.
  • Documentation matches the implemented schema and security model.
