Replace first-bytes diff truncation with explicit, deterministic coverage handling.

Scope:

  • Prioritize manually written and security-sensitive files.
  • Exclude configured generated/vendor paths.
  • Batch files within context limits and synthesize results.
  • Report exactly which files/batches were reviewed or omitted.

Acceptance:

  • No arbitrary silent diff truncation.
  • Final comments identify incomplete coverage and reason.
